Frequently Asked Questions:
1) Do I need a referral from my Family Doctor in order to make an appointment?
We do accept self-referrals, however it will depend on what insurance coverage you have as to whether or not you need a referral. If a referral is needed you must have it in place BEFORE you make an appointment.


2) Do I have to pay my co-pay at the time of service?
YES, co-payments are expected to be paid at the time of your office visit.


3) Is there anything I need to bring to my appointment?
Yes, please bring your current insurance card and/or Worker’s Comp information, a list of your current medications and any x-rays you have had taken.


4) Will there be any paperwork that I will need to fill out for my office visit?
Yes. If this is your first visit with us, there will be patient history forms to complete, as well as patient information forms to fill out. We then update all patient information yearly. You may access our forms online by clicking here.


5) Where are you located?
We have four office locations. Please reference our LOCATIONS section of the web site for the address and a map of the clinic you need.


6) Is my insurance accepted at your office?
Click here for a list of the insurance companies that we contract with. If your insurance company is not on this list please call the number on your insurance card to check, as insurance plans are always changing.


7) When I call the office with a question, can I speak to the doctor or will I speak to a nurse?
Normally when the doctors are in the office, they are seeing patients. We do have a nurse available to answer your questions, however, if she is busy answering other patient’s questions you will be connected to her voice mail and she will return your call as soon as she can.


8) Do you take Worker’s Comp cases?
Yes, however, it is important that all of the forms needed be completed. We will also take a copy of your regular insurance card in case there is a problem with the Worker’s Compensation claim.


9) How do I file a Worker’s Comp claim?
There is a form called First Report of Injury that needs to be filled out by the injured worker, their employer and the physician. Then this form is sent to the Bureau of Worker’s Compensation.
